Wellness Policy

Special Diets
Kerrie Zigler, the registered dietitian for the Deer Valley Unified School District, is available to work with parents and students to accommodate special diets for medical purposes. Special diets include diabetic diets, gluten free diets for celiac disease and PKU. Please contact Kerrie for more information at 623-445-5165 or email Kerrie.
Online Payment
Deer Valley Unified School District continues to offer EZ School Pay, an online account service that allows parents to add payments directly to the student's school meal account using Visa or MasterCard for a fee of $2.00 per transaction. Payments are posted on a secure website and will be received in the school cafeteria's computer system within minutes. Sign-up is quick, easy and secure. Parents can access the website at www.EZSchoolPay.com.
For instructions on creating an account, refer to the EZ School Pay Parent User Manual. It may be necessary for you to re-add your student if they make a change from one school to another within our district. If your student has changed school location; you may notice they have disappeared from your account or they are “grayed-out”. If this is the case, it will be necessary for you to re-add your student to your account by entering the student ID number and choosing the current school they are attending.

Meal Assistance Program
Applications for free and reduced priced meals are now available to all enrolled students in the Deer Valley Unified School District for the 2011-12 school year. Applications are available at each schools front office and cafeteria, online on this webpage to the right of this paragraph and at the Food & Nutrition Department at 21421 N. 21st Ave., Building 2, Phoenix, AZ 85027. All students who qualify for free or reduced meals are eligible to eat breakfast AND lunch for free or at a reduced price during the entire school year. Students are able to eat at any station in the cafeteria and order a “combo meal”, but students receiving meal benefits do not qualify for a la carte items. An account must be established in order to purchase a la carte items.
Students who participated in the Meal Assistance program last school year are eligible for the same benefits for the first 30 days of school for this school year. Re-applications are due before September 26, 2011. You must re-apply prior to this date to continue to receive benefits if you are eligible. Students eligibility to receive free or reduced meals remains confidential during meal service. We now have an online application.
Completed applications can be mailed or dropped off at the Food & Nutrition Department, located at 21421 N. 21st Ave., Building 2, Phoenix, AZ 85027. Applications can also be hand delivered to the school’s cafeteria manager. Only one application is required per family. Written notice of the students eligibility to participate in the Meal Assistance Program will be given to families within 10 working days of submitting an application.

Check Cashing Payment
The Food & Nutrition Department will accept personal checks only if a current name, address and phone number appear on the check. Nexcheck will be used to collect for non-sufficient funds checks. A $25.00 fee will be charged for collection of any non-sufficient funds issued to the Food & Nutrition Department.
